You’ll want to start by identifying a system-level patient safety concern for your task topic. When we say systems-level problem, we mean one that occurs repeatedly, affects more than one patient and is not the fault of any one individual but is instead a problem with a policy, procedure, process, etc. Systems refer to a hospital, facility, clinic, unit, etc.
Some common systemic patient safety concerns used for topics are patient falls, med administration errors, HAIs, HAPIs, patient misidentification, wrong site surgeries, unsafe staffing levels, etc. You can also speak to your management/ Quality Improvement Department to see if there are any patient safety quality improvement projects they are currently working on to get some ideas.
